I acknowledge that I am encouraged to advise my doctor and will continue to be monitored by my doctor throughout the duration of the Plant Lady Juice Co. Wholistic Consultation.

I understand that the Plant Lady Juice Co. Wholistic Consultation is not designed to be a substitute for any medication which may have been prescribed by a physician or would be prescribed if I were to seek medical attention.

I acknowledge that the Plant Lady Juice Co. Wholistic Consultation does not provide a medical
diagnosis, prognosis, or substitute for medication or medical advice.
